Home Health Registered Nurse (RN)

Trinity Health At Home – Grand Rapids is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes throughout the Great Grand Rapids area. In this role, you will create individualized care plans, administer medications, educate patients and families, and support recovery and independence. This position offers the opportunity to practice at the top of your license with greater flexibility, autonomy, and meaningful one-on-one patient relationships.

Responsibilities

• Deliver skilled nursing care in patients' homes
• Create individualized care plans
• Administer medications
• Educate patients and families about their care and recovery
• Support patient recovery and independence
• Coordinate care with the interdisciplinary team
• Maintain efficient documentation using the Epic EHR system

Schedule

• Full-time, day shift position
• Predictable schedules with steady assignments
• No on-call requirements

Required Qualifications

• Graduate of an approved nursing education program (BSN preferred)
• Current Registered Nurse licensure in the State of Michigan
• Minimum of one year of professional registered nursing experience
• Current driver's license and reliable transportation
• Strong computer and technology skills

Preferred Qualifications

• Medical-surgical nursing experience
• Home health or hospice experience
