Home Health Registered Nurse (RN)

As a Home Health Registered Nurse, you will provide personalized nursing care to help patients recover from illness, manage chronic conditions, and maintain independence in the comfort of their own environment. You will work with patients in private residences, assisted living facilities, and adult foster homes.

Responsibilities

• Provide skilled nursing care to patients in their homes
• Perform comprehensive patient assessments and develop individualized care plans
• Monitor patient progress and adjust care plans to support optimal outcomes
• Educate patients and families on disease management, medications, and recovery
• Collaborate with physicians and interdisciplinary teams to coordinate care
• Maintain accurate, timely documentation of assessments, treatments, and outcomes
• Advocate for patients and support their safety, comfort, and well-being

Required Qualifications

• Current RN license in Oregon
• BLS certification
• Minimum 1 year of RN experience in acute care
• Valid Oregon driver's license
• Reliable vehicle and current auto insurance

Preferred Qualifications

• Home Health, Community Health, or Critical Care experience
• Bachelor of Science in Nursing (BSN)

Schedule

• Monday through Friday, 8:00 AM to 4:30 PM
• Rotating weekend coverage shared among clinicians
• Maximum 40 hours per week
