Home Health Registered Nurse (RN) – Talent Pool

Optimal Care is seeking a Home Health Registered Nurse to deliver comprehensive skilled nursing care to patients in their homes. In this role, you will support patient recovery, chronic disease management, and health goals while helping individuals remain safely in their own environment. You will manage a caseload of patients from admission through discharge, providing skilled nursing care, conducting assessments, and developing individualized care plans. Working closely with physicians and an interdisciplinary care team, you will coordinate services, make informed clinical decisions, and play a central role in shaping patient outcomes.

Clinical Setting

You will provide care to patients in their homes, assisted living facilities, and nursing homes across a multi-county service area including Eaton, Ingham, Jackson, Washtenaw, Livingston, Hillsdale, Lenawee, and Monroe counties. The office location is in Jackson, Michigan.

Responsibilities

• Deliver comprehensive skilled nursing care to patients in home and facility settings
• Conduct patient assessments and develop individualized care plans
• Manage a caseload of patients from admission through discharge
• Coordinate services with physicians and interdisciplinary care team members
• Make informed clinical decisions and provide OASIS documentation
• Support patients with chronic disease management and recovery goals
• Travel daily within the service area to provide patient care

Schedule and Employment Type

Full-time positions are Monday through Friday. Part-time and per diem options are available for three weekdays per week. This is a high travel position requiring daily travel within the service area.

Required Qualifications

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum one year of experience as a licensed RN in a clinical care setting
• Reliable transportation, valid driver's license, and valid automobile insurance coverage
• Willingness to travel daily for home visits within service area

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Experience in home health environment
• OASIS certification or willingness to obtain

Compensation and Benefits

Salary range: $75,000 to $82,000 USD
