Home Health Registered Nurse (RN) - Spring/Woodland/Magnolia

Pathfinder Home Health is seeking a Full-Time Registered Nurse (RN) to provide skilled nursing care in patients' homes throughout the Spring, The Woodlands, and Magnolia, TX areas. This role leverages AI-driven technology through the Roger app, a voice-enabled, HIPAA-compliant documentation platform designed to reduce charting by 50% while maintaining OASIS compliance. You will spend over 90% of your time on direct patient care rather than paperwork.

Clinical Setting

You will deliver comprehensive skilled nursing care in home health settings, primarily serving geriatric patients. The role involves working with an interdisciplinary care team and requires a valid driver's license for travel between patient homes.

Responsibilities

• Provide high-quality, comprehensive nursing care in patients' homes according to physician orders and individualized plans of care
• Partner with the interdisciplinary care team to develop, implement, and update personalized care plans addressing each patient's unique needs
• Administer nursing treatments and manage clinical activities to support optimal health outcomes
• Continuously assess patient progress and the effectiveness of services
• Communicate findings with the care team, patient, family, physician, and case manager
• Perform additional duties as needed

Required Qualifications

• Active RN License (Texas or Compact)
• Active CPR Certificate
• Prior experience in geriatric home health care
• Valid Driver's License
• Self-starter with strong organizational skills
