Home Health Registered Nurse (RN) | Snohomish County

Alpha Home Health & Hospice is hiring a full-time Home Health Registered Nurse (RN) for the Whatcom County area. In this role, you will provide direct patient care in home settings, focusing on patients with complex medical needs. You will work autonomously to manage your own schedule while delivering high-quality nursing care and building meaningful relationships with patients and their families.

Clinical Setting

You will conduct home visits to provide one-on-one patient care in the home environment. This position offers flexible scheduling and the autonomy to manage your own caseload.

Responsibilities

• Conduct initial and ongoing patient assessments in the home setting
• Develop and update individualized care plans in collaboration with the care team
• Administer medications, treatments, and injections as prescribed
• Perform wound care and dressing changes and other complex care procedures
• Monitor vital signs and symptoms, reporting changes to the physician
• Educate patients and caregivers on disease processes, medications, and self-care
• Coordinate care with physicians, therapists, and other healthcare professionals
• Document all visits and care provided in the electronic medical record (EMR)
• Ensure compliance with agency policies, Medicare/Medicaid, and other regulations
• Participate in case conferences and interdisciplinary team meetings
• Provide emotional support and promote patient independence

Required Qualifications

• Active Washington RN licensure
• Current BLS certification
• Valid driver's license and insured vehicle

Preferred Qualifications

• Home health or hospice agency experience
• The employer is considering new RN graduates

Compensation and Schedule

• $45-$55 per hour, depending on experience
• Competitive per visit rates
