Home Health Registered Nurse (RN) | Snohomish County

Alpha Home Health & Hospice is hiring a full-time Home Health Registered Nurse (RN) for the Whatcom County area Home Health division. In this role, you will provide direct, one-on-one patient care in home settings, focusing on patients with complex medical needs. You will have autonomy in managing your own schedule while working collaboratively with a supportive team of healthcare professionals.

Responsibilities

• Conduct initial and ongoing patient assessments in the home setting
• Develop and update individualized care plans in collaboration with the care team
• Administer medications, treatments, and injections as prescribed
• Perform wound care and dressing changes and other complex care procedures
• Monitor vital signs and symptoms, reporting changes to the physician
• Educate patients and caregivers on disease processes, medications, and self-care
• Coordinate care with physicians, therapists, and other healthcare professionals
• Document all visits and care provided in the electronic medical record (EMR)
• Ensure compliance with agency policies, Medicare/Medicaid, and other regulations
• Participate in case conferences and interdisciplinary team meetings
• Provide emotional support and promote patient independence

Required Qualifications

• Active Washington RN licensure
• Current BLS certification
• Valid driver's license and insured vehicle

Preferred Qualifications

• Home Health or Hospice agency experience
• New RN graduates are being considered

Compensation and Schedule

• $45-$55 per hour, depending on experience
• Competitive per visit rates
