Home Health Registered Nurse (RN)

Sanzie Healthcare Services is hiring Registered Nurses for home health care services in Clayton County. This position involves providing skilled nursing care, conducting patient assessments, developing and revising care plans, and supervising personal support aides and licensed practical nurses in patients' homes. You will work with complex patient populations, coordinate care with other providers, and ensure comprehensive documentation of all clinical activities.

Clinical Setting

This is a home-based position serving patients through the Clayton County Select Plan (CCSP) and related Medicaid waiver programs. You will conduct face-to-face visits in patients' homes to evaluate care needs, monitor patient status, and supervise support staff.

Responsibilities

• Monitor and supervise care for patients with complex health needs and observations
• Review referrals from CCSP care coordinators and evaluate patient needs during initial and subsequent visits
• Develop, revise, and implement individualized member care plans
• Conduct face-to-face supervisory visits in patients' homes at least annually to observe and monitor the performance of personal support aides
• Supervise personal support aides and licensed practical nurses
• Complete and maintain clinical records, including progress notes and supervisory visit documentation
• Review, sign, and date all clinical record entries made by assigned support staff
• Review and co-sign all LPN supervisory visit documentation within ten days of each visit and follow up on any concerns
• Communicate patient care needs with CCSP providers, patients, family members, caregivers, and care coordinators
• Make appropriate recommendations to CCSP care coordinators regarding patient services
• Educate patients, family members, and caregivers about health status, level of functioning, and healthcare needs
• Report changes in patient status or condition to the patient's physician
• Arrange orientation and in-service training for staff

Required Qualifications

• Current, unencumbered Georgia Registered Nurse license
• Minimum two years of experience in home health services or a related field
• BLS certification
• Ability to supervise personal support aides
• Ability to communicate effectively
• Working knowledge of Rules and Regulations for Private Home Care Providers
• Working knowledge of CCSP, SOURCE, ICWP, and GAPP Medicaid Waiver Program policies and procedures
• Negative TB test (certificate required)
• Valid driver's license
• Professional liability insurance (such as NSO)
• Current proof of auto insurance
• Reliable transportation and personally owned vehicle
• COVID vaccine or screening documentation
• Physical examination clearance
• Photocopy of Social Security card

Preferred Qualifications

• Experience working with older populations

Education

• License in the Practice of Nursing
• Specialized training required for all personal care

Compensation

• Intake/admissions visits: $70 per visit
• Supervisory visits: $50 per visit
