CVHCare is seeking a Registered Nurse for home health clinical services in the San Jose area. As a Home Health RN, you will monitor patients' overall health and develop individualized plans of care in partnership with primary care physicians. You will function as a case manager, working independently to deliver quality healthcare in patients' homes while managing your own schedule and visit planning.
This is a field-based position requiring reliable transportation to visit patients in their homes. This is not a remote position.
Responsibilities
- Monitor patients' overall health status and develop individualized care plans
- Provide customized patient education about disease management
- Deliver specialized services including simple to complex wound therapy, post-surgical care management, and ostomy care management
- Function as a case manager and work independently with patients
- Coordinate with the primary care physician and interdisciplinary team
Clinical Setting
You will provide care in patients' homes across the San Jose area, including San Jose, Morgan Hill, Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, and Los Gatos.
Schedule
- Full-time position (24 RVE per week)
- Non-traditional schedules available, such as Thursday through Monday
- Flexibility to plan patient visits around other commitments
Required Qualifications
- Minimum one year of RN experience, preferably in acute care, skilled nursing facility, home health, or hospice settings
- Active California Registered Nursing License
- Associate degree (AA), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN)
- Reliable transportation
- Current CPR or BLS card
Training
You must be able to complete five days of paid onsite training Monday through Friday, 9 a.m. to 5:30 p.m. in San Ramon.

Compensation is based on full-time schedule.
