Home Health Registered Nurse (RN) - San Francisco

CVHCare is seeking a Registered Nurse for weekend home health visits in the San Francisco Bay Area. As a Home Health RN, you will monitor patients' overall health and develop individualized plans of care in partnership with primary care physicians. You will function as a case manager, working independently to deliver quality healthcare in patients' homes.

You will provide customized care and patient education about disease management, along with specialized services including simple to complex wound therapy, post-surgical care management, and ostomy care management. CVHCare provides comprehensive internal operational support to handle medication reconciliation, DME orders, MD signatures, coding, and lab follow-up, allowing you to focus on direct patient care and maintain flexibility in scheduling.

Schedule

This is a full-time or part-time position with a required commitment to work up to two weekends per month. Full-time positions include 24 visits per week with benefits. Part-time positions include 18 visits per week. Non-traditional schedules are available, such as Thursday through Monday. This is not a remote position.

Locations

Area 9 covers San Francisco. Area 10 covers San Mateo, Brisbane, Burlingame, Daly City, Redwood City, Hillsborough, Millbrae, Colma, South San Francisco, Pacifica, Belmont, San Bruno, and Half Moon Bay.

Training

You must be able to complete five days of paid onsite training Monday through Friday, 9 a.m. to 5:30 p.m., in San Ramon.

Required Qualifications

• Minimum one year of RN experience, preferably in acute care, skilled nursing facility, home health, or hospice settings
• Active California Registered Nursing License
• Associate degree (AA), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN)
• Reliable transportation
• Current CPR or BLS card
