Home Health Registered Nurse (RN) -Richmond/Oakland

CVHCare is seeking a Registered Nurse for weekend home health visits in the Richmond and Oakland areas. As a Home Health RN, you will monitor patients' overall health and develop individualized plans of care in partnership with primary care physicians. You will function as a case manager, working independently to deliver quality healthcare in patients' homes while providing patient education about disease management.

This position offers the flexibility to make your own schedule and plan patient visits around other commitments. You will have access to an internal operational support team that handles medication reconciliation, DME orders, MD signatures, coding, and lab follow-up, allowing you to focus on direct patient care.

Responsibilities

• Monitor patients' overall health and develop individualized care plans
• Provide direct patient care in home settings
• Deliver specialized services including simple to complex wound therapy, post-surgical care management, and ostomy care management
• Provide patient education about disease management
• Function as a case manager and work independently
• Coordinate with primary care physicians and support teams

Clinical Setting

This is a field-based position serving patients in their homes across the Richmond and Oakland areas, including the cities of Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont.

Schedule

• Full-time: 24 visits per week with benefits, or Part-time: 18 visits per week
• Must be available to work up to two weekends per month
• Non-traditional schedules available (for example, Thursday-Monday)
• This is not a remote position

Required Qualifications

• Minimum one year of RN experience, preferably in acute, SNF, home health, or hospice settings
• Active California Registered Nursing License
• Associate degree (AA), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN)
• Reliable transportation
• Current CPR or BLS card

Training

• Five-day paid onsite training required in San Ramon, Monday-Friday, 9 am to 5:30 pm
