This home health provider is seeking a Registered Nurse to deliver physician-prescribed home health care services to homebound, disabled, and chronically ill patients across Washington D.C. As a Home Health RN, you will make individualized patient-centered clinical decisions, develop customized care plans, and help patients and their caregivers achieve health goals and independence in their own homes. This is a full-time position working 40 hours per week in the field, providing direct patient care in patients' homes.
Clinical Setting
You will work for the largest home health care provider in the Maryland, Washington D.C., and Virginia region. The organization has been recognized as a HomeCare Elite agency for 10 consecutive years and ranked in the top 25% of agencies nationally. You will be assigned a centralized territory grouped by zip codes to minimize travel time while serving patients across Northwest or Southeast D.C.
Responsibilities
- Develop and document individualized plans of care with specific interventions and measurable patient outcomes
- Evaluate patient progress toward goals and revise care plans as needed
- Establish, document, and implement personalized teaching plans for patients and caregivers to promote independence at home
- Perform nursing procedures consistent with home care protocols using standard home care supplies and equipment
- Initiate and participate in regularly scheduled case conferences with all disciplines
- Include patients and families in mutual goal setting and care plan revision
- Maintain clinical records, collect data, and prepare reports on activities
- Communicate regularly with patients' physicians, the home health team, and other care team members
Required Qualifications
- 1-2 years of RN experience within the last 5 years in ICU, CCU, Med-Surg, Oncology, or Gerontology
- Valid RN license in the District of Columbia, Maryland, Virginia, or any combination based on work location(s)
- CPR certification
- Valid driver's license with an acceptable driving record and reliable transportation
Preferred Qualifications
- I.V. therapy skills
- Wound care experience
- Basic computer skills
Schedule
- Monday through Friday, 8:00 AM to 4:30 PM
- No on-call duties
- Weekend rotation every 3-4 weeks (responsible for admissions)
Productivity Requirements
- 27 points per week based on the following: Admission/recertification/resumption = 2 points; Revisit/discharge = 1 point; Additional clinical evaluation = 1.5 points
Compensation and Benefits
- Salary: $89,856 to $125,000 per year
