Home Health Registered Nurse RN PRN Part time

Aveanna is seeking a Registered Nurse for Home Health to provide skilled nursing care in patients' homes across the Lehigh, Fort Myers, and Cape Coral territories. In this PRN part-time role, you will develop individualized care plans, educate patients and families, coordinate care with physicians and other caregivers, and deliver a range of clinical services including patient assessment, wound care, infusions, catheter care, and PICC line dressing changes.

Schedule

• Monday through Friday with one weekend on-call rotation

Responsibilities

• Develop plans of care based on physician orders, client input, and nursing assessment, including medical interventions and measurable goals and outcomes
• Educate clients and family members based on specific clinical needs
• Orient and train primary caregivers to optimize client functioning
• Coordinate hands-on client care through documentation and timely communication with physicians and other caregivers
• Provide skilled nursing services in patients' homes, including phone triage, patient education, observation and assessment, wound care, infusions, catheter care, and PICC line dressing changes

Required Qualifications

• Active RN license in the state of application
• Valid CPR certification

Preferred Qualifications

• Medicare Skilled Nursing experience
• Basic understanding of OASIS
• One year of RN experience in a healthcare setting
• Homecare Homebase experience
