Home Health - Registered Nurse RN - PRN

NHC HomeCare Springfield is seeking a Registered Nurse (RN) for a PRN (as-needed) home health position. In this role, you will provide skilled nursing care to patients in their homes, utilizing the nursing process to identify and achieve patient health goals.

Responsibilities

• Assess patients' physical, psychosocial, and environmental factors to develop comprehensive nursing care plans that achieve desired health outcomes in a culturally appropriate manner
• Implement the nursing process including assessment, planning, implementation, and evaluation
• Collaborate with the interdisciplinary team to ensure personal care, medical care, and rehabilitation support optimal patient health and well-being
• Teach patients and caregivers about health care and disease management appropriate to their needs and level of understanding
• Document patient and caregiver understanding and continuing educational needs

Required Qualifications

• Unencumbered current Registered Nurse license in the state where the home care agency is located or compact state, if applicable
• Minimum one year of experience as a Registered Nurse or professional nurse

Preferred Qualifications

• Experience in home care
• RN training background
