We are seeking experienced Registered Nurses to provide home health care services to patients in their homes and facilities. This is a per diem position offering flexible scheduling and the opportunity to work independently while delivering comprehensive nursing care.
Responsibilities
- Regularly re-evaluate patient nursing needs and adjust care accordingly
- Initiate and revise the plan of care as patient status and needs change
- Use health assessment data to determine nursing diagnoses
- Develop care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions
- Include patients and families in the planning process
- Initiate appropriate preventive and rehabilitative nursing procedures
- Administer medications and treatments as prescribed by the physician
- Counsel patients and families on meeting nursing and related needs
- Provide health care instructions to patients as appropriate per assessment and plan of care
- Identify discharge planning needs and implement prior to patient discharge
- Prepare clinical notes and update the primary physician when necessary
- Communicate with physicians regarding patient needs and report changes in patient condition
- Obtain and receive physician orders as required
- Communicate with community health-related persons to coordinate the care plan
- Participate in on-call duties as defined by the on-call policy
- Ensure arrangements for equipment and other necessary items and services are available
Required Qualifications
- Minimum of 2 years of home health care experience
- Current RN license in Illinois
- Current CPR certification
- Excellent observation, verbal, and written communication skills
- Problem-solving skills and basic math skills
- Nursing skills per competency checklist
- Valid driver's license with a personally owned, insured automobile in good working order
- Ability to perform prolonged or considerable walking or standing
- Ability to lift, position, and transfer patients
- Ability to lift supplies and equipment
- Ability to perform considerable reaching, stooping, bending, kneeling, and crouching
- Visual acuity and hearing sufficient to perform required nursing skills
Compensation
- Standard Visit: $72.00
- Recertification, Resumption, and Discharge OASIS: $82.00
- Start of Care OASIS: $105.00
- Start of Care OASIS (Weekend): $115.00
- Mileage Reimbursement: $0.60 per mile after 15 mile per day deduction
