Home Health Registered Nurse RN

This position provides skilled nursing care to clients in their homes under physician orders. As a Home Health Registered Nurse, you will assess patient health problems and needs, develop and implement individualized nursing care plans, and maintain comprehensive medical records. You will work collaboratively with physicians, therapists, and other members of the healthcare team to coordinate patient care and provide education to clients and families on proper home health care procedures.

Responsibilities

• Provide skilled nursing treatments as ordered by the client's physician
• Complete evaluation tasks, including vital signs and medication reviews
• Assess and chart observations of the client's condition at each visit
• Maintain complete and accurate documentation of nursing care
• Coordinate with physicians, therapists, and other individuals in the client's care plan
• Work collaboratively with all members of the healthcare team
• Provide education to clients and families on proper home health care procedures and strategies
• Record patients' medical information and vital signs
• Monitor, record, and report symptoms or changes in patient's conditions
• Administer local, inhalation, intravenous, or other anesthetic as ordered
• Prepare patients for and assist with examinations or treatments
• Modify patient treatment plans as indicated by patients' responses and conditions
• Order, interpret, and evaluate diagnostic tests to identify and assess the patient's condition
• Monitor all aspects of patient care, including diet and physical activity
• Engage in research activities and participation in formal educational training activities related to nursing
• Perform other duties as assigned

Schedule

• Available Monday, Wednesday, and Friday

Required Qualifications

• Associate's degree in nursing (Bachelor's degree from an accredited school of nursing preferred)
• Current and valid Connecticut Registered Nurse License
• Current CPR certification
• Valid driver's license, car insurance, and car registration
• Strong interpersonal skills that build personal rapport and establish trust with clients
• Computer literacy
• Strong organizational skills
• Excellent written and communication skills
• Ability to meet physical demands of the job, including lifting up to 50 pounds for patient transfers

Preferred Qualifications

• Prior nursing experience, specifically in home health
• Home health care experience
• Bilingual proficiency in Spanish

The employee is regularly required to talk or hear. The employee frequently is required to use hands or fingers and handle or feel objects, tools, or controls. The employee is occasionally required to stand, walk, sit, and reach with hands and arms. The employee must occasionally lift and move up to 50 pounds. Specific vision abilities required include close vision, distance vision, and the ability to adjust focus. The noise level in the work environment is usually low to moderate.
