Home Health Registered Nurse (RN) | One-on-One Patient Care

SummitWest Care, a nonprofit home health agency serving Mesa County, Colorado, is seeking a Registered Nurse (RN) to provide skilled, patient-centered care in patients' homes. This position offers a different nursing experience focused on meaningful one-on-one patient relationships rather than high-volume patient assignments. You will have the clinical autonomy to structure your day while receiving support from an experienced interdisciplinary team.

Clinical Setting

Home health nursing in Mesa County, Colorado. You will work independently in patients' homes, providing direct skilled nursing care and building lasting relationships with patients and families.

Responsibilities

• Perform comprehensive patient assessments
• Develop and update individualized plans of care
• Provide skilled nursing care and appropriate interventions
• Monitor patient progress and communicate changes to the care team
• Educate patients and families regarding medications, treatments, and care plans
• Coordinate care with LPNs, therapists, caregivers, and providers
• Complete timely and accurate EMR documentation
• Participate in on-call rotation

Required Qualifications

• Active Registered Nurse (RN) license in the State of Colorado or active NLC Compact License
• Valid driver's license, reliable transportation, and auto insurance
• Strong assessment, clinical judgment, and critical thinking skills
• Ability to work independently and manage a structured caseload
• Professional communication skills with patients, families, and team members
• Comfort with documentation and EMR systems

Preferred Qualifications

• Home health experience
• Experience with HCHB EMR system

Schedule

Full-time or part-time positions available. Monday through Friday day schedule with on-call rotation participation required.

Compensation

$35–$43 per hour, depending on experience. Paid drive time and mileage reimbursement included.
