Home Health Registered Nurse (RN)

One Home Health is seeking an experienced Registered Nurse to join our home health team in Elgin, IL. This is not an entry-level position. You will manage a caseload of home health patients, completing skilled nursing visits, assessments, and care plan updates in patients' homes. You must be comfortable working independently without on-site supervision and be able to think critically and respond to changes in patient status.

Responsibilities

• Manage a caseload of home health patients and complete skilled nursing visits, assessments, and care plan updates in the field
• Perform admissions, recertifications, and discharges with accurate and timely OASIS documentation
• Coordinate with physicians, therapists, and the interdisciplinary team on changes in patient condition
• Educate patients and family caregivers on disease management, medications, and home safety
• Respond to changes in patient status and escalate appropriately
• Maintain documentation that meets Medicare, state, and agency compliance standards

Required Qualifications

• Active, unencumbered RN license in Illinois
• Minimum of 1-2 years of RN experience with home health experience required
• Working knowledge of OASIS documentation and Medicare home health guidelines
• Comfortable working independently in patients' homes across the service area
• Reliable transportation and valid driver's license
• Strong clinical judgment and ability to prioritize a full caseload without close supervision

Preferred Qualifications

• Experience with Axes
• Certification in wound care, oncology, or chronic disease management
• Bilingual candidates encouraged to apply
