Home Health Registered Nurse (RN) - Hayward

CVHCare is seeking a Registered Nurse for full-time home health clinical services in the San Francisco Bay Area. As a Home Health RN, you will monitor patients' overall health and develop individualized plans of care in partnership with primary care physicians. You will function as a case manager, working independently to deliver quality healthcare in patients' homes while maintaining flexibility to schedule visits around other commitments.

This is a field-based position, not remote. You will provide direct patient care including simple to complex wound therapy, post-surgical care management, ostomy care management, and patient education about disease management. An internal operational support team handles medication reconciliation, DME orders, MD signatures, coding, and lab follow-up, allowing you to focus on clinical care and patient interaction.

Clinical Setting

Home health care in the patient's home environment. Service areas include:

• Area 1: Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City
• Area 3: San Ramon, Dublin, Pleasanton, Sunol, Livermore

Schedule

• Full-time position: 24 RVE per week
• Must be available to work up to two weekends per month
• Non-traditional schedules available (for example, Thursday-Monday)
• Paid training required: 5 days Monday-Friday, 9am-5:30pm in San Ramon (onsite)

Required Qualifications

• Minimum 1 year of RN experience, preferably in acute care, skilled nursing facility, home health, or hospice setting
• Active California Registered Nursing License
• Associate degree (AA), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN)
• Reliable transportation
• Current CPR or BLS certification
