Home Health Registered Nurse - RN - Greater Kansas City Metro Area

Lux Home Health is seeking a Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the Greater Kansas City Metro Area. In this role, you will conduct initial patient assessments, develop individualized care plans, deliver wound care and prescribed treatments, and collaborate with physicians, therapists, and family members to coordinate comprehensive patient care. You will play a vital role in helping patients maintain independence and improve their quality of life while managing their health in a home setting.

Responsibilities

• Perform thorough initial assessments and develop individualized care plans addressing each patient's unique needs and health goals
• Provide wound care and other treatments as prescribed by physicians
• Collaborate with physicians, therapists, and family members to ensure coordinated patient care
• Maintain accurate, up-to-date documentation of all patient care activities in accordance with company policies and regulatory standards
• Educate patients and families on managing medical conditions, taking medications, and navigating overall health
• Respond promptly to medical emergencies and patient concerns, delivering professional care and appropriate interventions

Required Qualifications

• Active Registered Nurse (RN) license in Missouri
• Minimum 2 years of home health nursing experience
• Current CPR certification
• Strong clinical skills and experience providing skilled nursing care in a home setting
• Ability to work independently, manage a patient caseload, and prioritize tasks effectively
• Valid Missouri driver's license, reliable transportation, and ability to travel within the service area
• Excellent communication, interpersonal, and organizational skills
