Home Health Registered Nurse (RN)

The Roads Home Health Care is seeking a Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout South Florida. Working alongside a Case Manager, you will deliver hands-on patient care, assess patient conditions, manage pain, administer medications, and coordinate multidisciplinary care for an assigned caseload.

Responsibilities

• Provide skilled nursing care in patients' homes following individualized plans of care
• Assess each patient's condition and adjust the plan of care as needed
• Assess and manage patients' pain using appropriate techniques and patient/family education
• Administer medications correctly based on patient age and clinical status, following best practices to prevent errors
• Obtain and follow physician orders, reporting changes in patient physical or emotional condition promptly
• Participate in case conferences to discuss patient progress and coordinate multidisciplinary care
• Teach, supervise, and counsel patients and family members on nursing care needs
• Complete accurate, timely documentation and charting for every visit

Required Qualifications

• Active Registered Nurse (RN) license in the State of Florida
• Current BCLS/CPR certification
• Minimum one year of acute care nursing experience
• Valid Florida driver's license, current automobile insurance, and reliable transportation

Preferred Qualifications

• Prior home health experience
• Bilingual capabilities
• Experience with home health EMR or documentation software

Additional Requirements

• Comfortable delivering patient care across the lifespan, from pediatric to geriatric patients
• Strong verbal and written communication skills for coordinating with physicians, patients, and families
• Basic computer proficiency for entering and managing patient documentation
• Working knowledge of federal, state, and agency regulations governing home health care
• Respectful, culturally sensitive approach to patient and family education
