Home Health Registered Nurse (RN) - Dublin

CVHCare is seeking a Registered Nurse for home health field visits in the San Ramon and East Bay areas. In this role, you will monitor patients' overall health and develop individualized plans of care in partnership with primary care physicians. You will function as a case manager, working independently to deliver quality healthcare in patients' homes while providing patient education about disease management.

You will provide a variety of specialized services including simple to complex wound therapy, post-surgical care management, and ostomy care management. CVHCare provides comprehensive internal operational support to handle medication reconciliation, DME orders, physician signatures, coding, and lab follow-up, allowing you to focus on direct patient care and maintain flexibility in scheduling your visits.

Clinical Setting

This is a field-based position serving patients in their homes across two service areas: Area 3 (San Ramon, Dublin, Pleasanton, Sunol, Livermore) and Area 4 (Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek). This is not a remote position and requires reliable transportation.

Schedule

• Full-time position: 24 RVE per week
• Part-time position: 18 RVE per week
• Must be available to work up to two weekends per month
• Non-traditional schedules available (for example, Thursday-Monday)
• Paid training required: 5 days Monday-Friday, 9am-5:30pm in San Ramon (onsite)

Required Qualifications

• Minimum 1 year of RN experience, preferably in acute care, skilled nursing facility, home health, or hospice settings
• Active California Registered Nursing License
• Associate degree (AA), Bachelor of Science in Nursing (BSN), or Master of Science in Nursing (MSN)
• Reliable transportation
• Current CPR or BLS certification
