The opportunity

Job Description

Tufts Medicine Care at Home is seeking a Home Health Registered Nurse and Case Manager to deliver comprehensive home health services to patients in their residences across the Salem, Hudson, Pelham, Nashua, and surrounding areas of New Hampshire. In this role, you will utilize the nursing process to manage patient assessments, interventions, and supportive care while emphasizing the prevention of hospitalization. You will serve as a key member of the healthcare team, interacting with patients, families, physicians, and community resources to facilitate safe and efficient care.

Responsibilities

  • Assess patients' physical and psychosocial needs in a sensitive, caring manner following established Standards of Nursing Practice and agency procedures
  • Assess patient and family learning needs, styles, and limitations, adjusting information delivery accordingly
  • Establish realistic goals and develop treatment plans in cooperation with patients, families, and healthcare team members
  • Complete physicians' orders, levels of care, and OASIS documentation for all assigned patients
  • Reconcile medications with patients and physicians consistently
  • Perform telephone triage, problem-solving, and patient/caregiver advocacy and support
  • Coordinate care and discharge planning with other team members during case conferences
  • Manage assigned caseload effectively within the team model of care delivery
  • Establish daily work plans based on patient and family priorities and total area needs
  • Promptly triage patient visits, messages, and phone calls according to priority and urgency
  • Demonstrate excellent physical assessment and care planning skills
  • Demonstrate current knowledge of pharmacology and medication administration
  • Provide appropriate supportive care to patients and families experiencing emotional stress
  • Utilize Security when needed for potentially unsafe situations
  • Communicate and collaborate with all disciplines in the home care setting on a regular basis or immediately for critical needs and crisis interventions
  • Maintain expert working knowledge of third-party payers, regulatory bodies, and agency policies related to documentation and care

Schedule

  • Full-time, 40 hours per week
  • Days shift
  • Weekend and holiday commitment required

Required Qualifications

  • Massachusetts RN licensure
  • Valid state-issued driver's license
  • One year of acute medical/surgical nursing experience
  • Cardiopulmonary Resuscitation (CPR) certification

Physical Requirements

  • Ability to stand, walk, sit, and use hands to handle objects and controls
  • Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl
  • Ability to talk and hear
  • Regular lifting and/or moving of 25 pounds; occasional lifting and/or moving up to 100 pounds
  • Close vision, distance vision, and ability to adjust focus
  • Frequent internal and external communication via phone, email, and in person
  • Ability to exchange accurate information
  • Exposure to moderate noise levels and weather conditions

Skills and Abilities

  • Knowledge and skills necessary to provide age-appropriate patient care
  • Competence in home health and hospice standards of care, symptom management, and psychosocial and spiritual aspects of care
  • Excellent interpersonal skills with ability to communicate, read, and write in English
  • Ability to function well, effectively manage time, and prioritize busy schedules
  • Good organizational skills
  • Ability to function effectively, autonomously, and independently in a home care setting

Compensation and Benefits

  • Base pay range: $84,300.22 - $107,481.20 (based on full-time equivalent)
