Aveanna Healthcare's Visiting Registered Nurse (VNA) Certified division is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes. You will serve as the primary clinical partner for patients and families, providing individualized, one-on-one nursing care that promotes recovery, safety, and independence. This role serves diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs. Services are covered by Medicare, Medicaid, Private Insurance, and Private Pay.
Responsibilities
- Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, and translate findings into patient-centered care plans
- Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management tailored to each patient's goals and condition
- Educate and empower patients and caregivers through clear instruction, coaching, and support to help them manage symptoms, medications, and daily care at home
- Monitor patient progress closely, identify changes in condition early, and communicate effectively with physicians and the interdisciplinary care team
- Coordinate care across providers and services to ensure continuity, clarity, and high-quality outcomes throughout the patient's home health journey
- Complete timely, accurate documentation that reflects the full clinical picture and supports quality, compliance, and measurable outcomes
Schedule and Compensation
- Flexible scheduling available: full-time, part-time, days, nights, and weekend shifts
- Pay is $45-50 per hour per visit based on the pay point model
- Ability to choose the radius you travel to
- Weekly pay

Required Qualifications
- Active, unencumbered RN license in the state of application (compact licenses must be transferred to your state of residence within 90 days)
- Current CPR certification with hands-on component
- Current TB skin test (within last 12 months)
- Valid driver's license, reliable transportation, and auto insurance
Preferred Qualifications
- Medicare Skilled Nursing experience
- Basic understanding of OASIS
- One year of RN experience in a healthcare setting
- Six months of prior hands-on nursing experience

- Variable hour and part-time employees are measured for eligibility into ACA Benefits plans over their first twelve months of employment.
