Home Health Registered Nurse (RN) (58692)

Interim HealthCare is hiring a Home Health Registered Nurse (RN) to provide direct patient care in the Stevens Point, Wisconsin area and surrounding communities. In this role, you will deliver one-on-one care to patients with illnesses, injuries, and chronic diseases, often following discharge from hospitals, rehabilitation facilities, or nursing homes. You will work autonomously while collaborating with physicians and supervising LPNs, CNAs, and aides.

Responsibilities

• Provide home-based nursing care to patients with acute and chronic conditions
• Conduct patient assessments, examinations, and documentation of progress
• Report clinical changes and findings to physicians
• Educate patients on their plan of care, prescribed medications, therapy, diet, and exercise
• Coach family members on patient care, medication management, and home safety
• Oversee and coordinate care provided by LPNs, CNAs, and aides
• Ensure treatment goals are met and coordinate discharge from services
• Maintain compliance with state and federal home health regulations

Required Qualifications

• Graduate of an accredited nursing program
• Active RN license in Wisconsin
• Minimum of 1 year of nursing experience
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty
• Strong communication skills

Preferred Qualifications

• Prior experience in hospice or home healthcare

Compensation and Benefits

• Base salary: $75,000 - $90,000
