Home Health Registered Nurse (RN): 12 Hour Shifts, PEG Tube, Trach, & Wound Care Experience

The Lakes Home Care is seeking a Licensed Registered Nurse for home health care. The patient is bedbound and requires skilled nursing care, including wound care in the sacral area, PEG tube feeding, and tracheostomy management. You will work in a one-on-one patient care setting with flexible scheduling that you create yourself.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
• Collect, analyze, and interpret data and information from health care members
• Document actual and potential nursing diagnoses
• Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
• Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards
• Revise the plan of care according to evaluation, changes in medical plan of care, and effectiveness of nursing interventions
• Use clinical judgment to establish priorities and meet patient care needs
• Perform other Registered Nurse duties as assigned

Required Qualifications

• Current Registered Nurse (RN) license for the state in which you practice
• Current Health Certificate (per facility contract or state regulation)
• Current PPD or Chest X-Ray
• Current BLS card

Preferred Qualifications

• One year of prior Registered Nurse experience
