Job Description

Residential Home Health and Hospice is hiring a Registered Nurse for home health positions in the Traverse City and Lapeer, Michigan areas. In this role, you will deliver skilled, patient-centered nursing care to patients in their homes, working as part of an interdisciplinary team while maintaining clinical autonomy.

Responsibilities

  • Perform comprehensive patient assessments and develop individualized care plans
  • Manage medications and provide skilled nursing interventions
  • Educate patients and families on disease management and treatment plans to promote confidence and informed decision-making
  • Think critically and solve problems in real time while ensuring thorough, compliant electronic medical record documentation
  • Travel to patient homes within your assigned territory, spending approximately 20% of your workday driving to and from patient locations
  • Work independently while collaborating with an interdisciplinary support team

Clinical Setting

This is a field-based home health position where you will provide care in patients' homes rather than a traditional clinical facility.

Schedule and Employment

  • Full-time employee position
  • 40 hours per week

Required Qualifications

  • Active Registered Nurse (RN) license in Michigan
  • Minimum one year of RN experience with demonstrated clinical expertise in delivering quality patient care
  • Valid driver's license and ability to drive to patient locations
