Residential Home Health and Hospice is hiring a Registered Nurse to provide skilled nursing care to patients in their homes in the Bay City, MI area. Responsibilities
- Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team
- Perform patient assessments, medication management, and care planning
- Educate and empower patients and families on disease management and treatment plans
- Think critically and solve problems in real time
- Ensure thorough, compliant EMR documentation
- Work independently while supported by your team
- Travel to patient homes within your assigned territory
Required Qualifications
- Active Registered Nurse (RN) license in the state where you work
- One year of RN experience with clinical expertise to deliver quality patient care
- Valid driver's license
- Ability to spend approximately 20% of your workday driving to and from patient locations
- Legal authorization to work in the United States

Compensation and Bonuses
