Home Health Registered Nurse

Residential Home Health and Hospice is hiring a Registered Nurse to provide skilled nursing care to patients in their homes in the Bay City, MI area. Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically and solve problems in real time
• Ensure thorough, compliant EMR documentation
• Work independently while supported by your team
• Travel to patient homes within your assigned territory

Required Qualifications

• Active Registered Nurse (RN) license in the state where you work
• One year of RN experience with clinical expertise to deliver quality patient care
• Valid driver's license
• Ability to spend approximately 20% of your workday driving to and from patient locations
• Legal authorization to work in the United States

Travel Requirements

• Field-based position requiring travel to patient homes within assigned territory
• Approximately 20% of workday spent driving to and from patient locations

Compensation and Bonuses

• First year success bonus of $10,000 to reward accomplishment of key achievements during your first year
