Home Health Registered Nurse

Remedy Home Health Care is hiring Registered Nurses for per diem home health visits in the West LA and surrounding areas. In this role, you will provide skilled nursing care directly to patients in their homes, including assessments, wound care, medication management, and treatments. You will develop and implement individualized care plans, monitor patient progress, educate patients and families on disease management and rehabilitation, and coordinate care with physicians, therapists, and the Director of Nursing.

Clinical Setting

You will work in patients' homes throughout the West LA area, providing community-based and senior care services.

Responsibilities

• Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments
• Develop and implement individualized home health care plans
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on disease management, rehabilitation, and senior care
• Coordinate care with physicians, therapists, and the Director of Nursing
• Document patient visits in compliance with home health regulations

Schedule

Per diem/PRN with flexible scheduling.

Required Qualifications

• Active California Registered Nurse (RN) license
• Minimum 1 year of RN experience
• Valid driver's license and reliable transportation
• Ability to travel throughout the West LA area

Preferred Qualifications

• Home health RN experience
• Public Health Nurse (PHN) certification
• Experience with mobile therapy or home-based care models
• Experience in rehabilitation, geriatric care, or community health
