Home Health Registered Nurse- PRN

Providence at Home with Compassus is seeking a Home Health Registered Nurse (RN) to provide expert care in patients' homes. This is a per diem, part-time position on day shift. As an RN, you will work both independently and as part of an interdisciplinary team to conduct comprehensive assessments, develop and implement individualized care plans, and deliver outcome-driven interventions under the physician-ordered Plan of Care. This collective bargaining position is rooted in the principles of community health nursing.

Responsibilities

• Conduct and document comprehensive patient assessments upon admission and continually reassess patients
• Apply appropriate advocacy and interventions as indicated by patient needs
• Interpret patient self-assessments and behavior with understanding of cognitive, physical, emotional, psychosocial, and developmental factors
• Collaborate with patients, families, and healthcare team members to develop, document, implement, and monitor individualized plans of care based on best practice and evidence-based medicine
• Provide legible and electronic documentation of patient care activities and information in an accurate, concise, and timely manner
• Support and assist with orientation of new staff and students
• Acquire necessary training and skills to competently perform preceptor and charge nurse functions when selected
• Demonstrate advancement of professional nursing practice through continuing education, required training, certification, and participation in department activities
• Foster open communication, patient and family-centered care, and healthy collegial relationships
• Demonstrate adaptive and timely communication based on individual patient and family needs
• Attend and participate in a minimum of 50% of scheduled staff meetings or read minutes and sign off on them

Required Qualifications

• Associate or Bachelor's degree from an accredited School of Nursing
• Current, unencumbered Registered Nurse license in the state of employment
• National Provider BLS certification from American Heart Association (vendor managed)

Preferred Qualifications

• One year of recent experience in home health, acute care, or skilled nursing facility

Physical Demands and Work Environment

This role requires regular standing, walking, and manual dexterity. You must be able to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.

Compensation and Benefits

Compensation range: $40.04 to $72.09 per hour. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. Compassus offers a comprehensive benefits package including medical, dental, vision, and 401k.
