Job Description

Ascension at Home, Together with Compassus, is seeking a Home Health Registered Nurse (RN) to provide clinical care and support to patients in their homes. In this role, you will help patients recover and maintain their ability to remain at home while spending quality time with loved ones and engaging in activities they enjoy.

Responsibilities

Support patient recovery and enable patients to stay at home
Educate patients and families on disease management to help them reach healthcare goals
Case manage and provide clinical oversight to the interdisciplinary team

Required Qualifications

Active RN license in the state where you work
Two years of experience as a registered nurse
Current driver's license
Current CPR and BCLS certification

Preferred Qualifications

Home health nursing experience
