Home Health Registered Nurse - Per Diem

InTouch Home Care Inc. is seeking a Registered Nurse for per diem home health visits in Santa Clara and San Mateo counties. This position is ideal for nurses looking to supplement income, pick up additional hours, or return to one-on-one patient-centered care without committing to set 8 or 12-hour shifts. You will visit patients at their residences and have flexibility to choose your own hours based on your schedule and patient availability.

Clinical Setting

InTouch Home Care Inc. is a growing provider of home health nursing and rehabilitation services across 9 counties in and around the San Francisco Bay Area. You will provide nursing services in patients' homes within Santa Clara and San Mateo counties.

Responsibilities

• Complete initial assessments of patients and families to determine home care needs, including comprehensive physical assessments and histories of current and previous illnesses
• Regularly re-evaluate patient nursing needs and revise plans of care as patient status changes
• Develop care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions, including patient and family in the planning process
• Use health assessment data to determine nursing diagnoses
• Initiate appropriate preventive and rehabilitative nursing procedures
• Administer medications and treatments as prescribed by the physician
• Draw blood or administer IVs as per patient needs
• Counsel patients and families in meeting nursing and related needs
• Provide health care instructions to patients as appropriate
• Identify discharge planning needs and implement prior to patient discharge
• Act as Case Manager and coordinate patient care for assigned caseload
• Prepare and complete clinical notes in a timely manner
• Communicate with physicians regarding patient needs and report changes in patient condition; obtain and receive physician orders as required
• Communicate with community health-related persons to coordinate care plans
• Maintain open communication via agency HIPAA-compliant messaging systems
• Instruct, supervise, and evaluate home health aide care provided every two weeks
• Instruct, supervise, and evaluate LVN care provided every month (RNs only)

Schedule

This is a per diem position paid on a per visit basis. You have flexibility to choose your hours and schedule as many or as few hours as suit your schedule, based on your availability and patient needs.

Required Qualifications

• Graduate of an accredited school of nursing
• One to two years of recent acute care experience in an institutional setting
• Current California RN licensure
• Current CPR certification
• Excellent observation, verbal, and written communication skills
• Problem-solving skills and basic math skills
• Nursing skills per competency checklist
• Ability to perform prolonged or considerable walking or standing
• Ability to lift, position, or transfer patients
• Ability to lift supplies and equipment
• Ability to perform considerable reaching, stooping, bending, kneeling, or crouching
• Visual acuity and hearing sufficient to perform required nursing skills
• Valid driver's license with a personally owned, insured automobile in good working order that meets state and organizational requirements
