Home Health Registered Nurse-Per Diem

CenterWell Home Health is seeking a Registered Nurse to provide skilled nursing care to patients in their homes. In this per diem role, you will work one-on-one with patients who are managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. You will report to the Clinical Manager and collaborate with physicians, therapists, social workers, and home health aides to develop and implement individualized care plans that support patient recovery and independence.

Responsibilities

• Provide skilled nursing care in patients' homes, including administering medications, performing wound care, and conducting patient assessments
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals
• Monitor patients' conditions and report changes to the patient's physician and Clinical Manager
• Educate patients and their families on disease management, medication, and treatment options
• Maintain accurate records of patient care and coordinate with other healthcare professionals
• Oversee the work of Home Health Aides and monitor their appropriate completion of documentation

Required Qualifications

• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Medical-surgical, ICU, ER, or acute care experience
• Experience collaborating with a team of healthcare professionals
• Valid driver's license, personal vehicle liability insurance, and reliable transportation

Preferred Qualifications

• One year of nursing experience
• Home health experience

Schedule and Employment

• Per diem position with 1 scheduled weekly hour
• This role requires driving to patients' homes in your personal vehicle

Compensation and Benefits

• Pay rate: $49.00 - $69.00 per visit/unit
• Annual salary range (full-time equivalent): $77,200 - $106,200 per year

Additional Requirements

• This is a patient-facing role and part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
• This role is part of Humana's driver safety program. You must maintain a valid state driver's license and personal vehicle liability insurance in accordance with your state's minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
