The opportunity

Job Description

Deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health goals. As a Home Health Registered Nurse, you will manage a caseload of patients from admission through discharge, providing individualized care in a role that emphasizes clinical independence and patient relationships.

You will conduct patient assessments, develop individualized care plans tailored to each patient's needs, and coordinate services with physicians and an interdisciplinary care team. Working with minimal supervision, you will make informed clinical decisions that shape patient outcomes and support each person's journey toward greater health and independence. Patients may be located in their homes, assisted living facilities, or nursing homes within the service area.

Clinical Setting

This is a high-travel position requiring daily travel within the Milford and surrounding area service area. Office location is 24255 W 13 Mile Rd, Suite 250, Bingham Farms, MI 48025.

Schedule

  • Monday to Friday, 8 a.m. to 5 p.m.
  • Full-time position

Required Qualifications

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse
  • Minimum 1 year of experience as a licensed RN in a clinical care setting
  • Reliable transportation, valid driver's license, and valid automobile insurance coverage
  • Willingness to travel daily for home visits within service area

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Experience in home health environment
  • OASIS certification or willingness to obtain
