Home Health Registered Nurse

Ascension Saint Thomas at Home, in partnership with Compassus, is seeking a Home Health Registered Nurse to support patient recovery and enable patients to remain in their homes while maintaining quality time with loved ones and engaging in activities they enjoy.

In this role, you will provide clinical care, case management, and interdisciplinary team oversight. You will empower patients and families to reach their healthcare goals through education on disease management and support for recovery.

Responsibilities

• Support patient recovery and ability to remain at home
• Educate patients and families on disease management
• Case manage and provide clinical oversight to the interdisciplinary team
• Coordinate care to help patients and families reach healthcare goals

Required Qualifications

• Active RN license in the state where you work
• Two years of experience as a registered nurse
• Current driver's license
• Current CPR and BCLS certification

• Home health nursing experience
