Home Health Registered Nurse

Interim HealthCare is hiring a Home Health Registered Nurse to provide direct patient care in the Green Bay, WI and surrounding area. In this role, you will deliver home-based nursing care to patients recovering from hospitalization, rehabilitation, or nursing facility stays, as well as those managing chronic illnesses and injuries. You will work autonomously while collaborating with physicians and supervising LPNs, CNAs, and aides to ensure comprehensive patient care and positive outcomes.

Responsibilities

• Provide direct home-based nursing care to patients with illnesses, injuries, and chronic diseases
• Conduct patient assessments, examine patients, document progress, and report clinical changes to physicians
• Oversee and coordinate care provided by LPNs, CNAs, and aides
• Educate patients on their plan of care, prescribed medications, therapy, diet, and exercise
• Coach family members on the patient's plan of care, medication management, and home safety
• Ensure treatment goals are met and coordinate discharge from services
• Maintain knowledge of state and federal home health regulations

Required Qualifications

• Graduate of an accredited nursing program
• Active RN license in Wisconsin
• Minimum of 1 year of nursing experience
• CPR certification
• Clinical proficiency in nursing specialty
• Strong communication skills
• Knowledge of state and federal home health regulations

Preferred Qualifications

• Prior experience in hospice or home healthcare

Compensation and Benefits

• Base salary: $75,000 - $85,000
