Home Health Registered Nurse

HealthCare California is seeking a Home Health Registered Nurse to provide skilled nursing care to patients in their homes throughout the Fresno, Merced, and Visalia areas. This position offers the opportunity to deliver one-on-one patient care in a supportive team environment with flexible scheduling and remote work capabilities.

Responsibilities

• Assess patient needs and initiate care planning
• Provide skilled nursing services based on individualized plans of care
• Prepare necessary items for treatments and procedures
• Implement infection prevention standards and instruct patients on medication management and safety
• Make treatment recommendations based on patient acuity level and goal potential
• Complete admissions processes, including same-day electronic documentation
• Remain flexible to accommodate admissions within time, travel, and geographic constraints
• Document skilled care on all visits according to agency standards
• Drive throughout the day to visit patients in their homes

Required Qualifications

• Valid California Registered Nurse license
• Minimum of 2 years of experience in Home Health or Acute Care nursing
• Current CPR certification
• Valid California Driver's License
• Computer skills, including typing proficiency for online documentation and reporting
• Willingness and ability to work with electronic input and telecommunication devices
• Excellent clinical skills and demonstrated time management ability
• Excellent communication skills, both verbal and written
