The opportunity

Job Description

This full-time home health position provides individualized, goal-directed nursing care to patients in their homes. You will manage comprehensive patient assessments, develop and implement plans of care, and evaluate progress toward health outcomes using the nursing process and principles of primary nursing.

Responsibilities

  • Collect comprehensive assessment data pertinent to patients' healthcare needs and formulate nursing diagnoses based on analysis of that data
  • Document assessment findings in a concise and timely manner
  • Develop and follow individualized plans of care that prescribe interventions to attain expected outcomes
  • Identify expected outcomes for the plan of care individualized to the patient, family, and situation
  • Implement and coordinate delivery of care while employing strategies to promote health and a safe environment
  • Evaluate progress toward attainment of outcomes
  • Systematically evaluate the quality and effectiveness of nursing practice
  • Collaborate with the family in the conduct of nursing practice, ensuring all decisions and actions are determined in an ethical manner
  • Integrate research findings into practice and participate in the generation of new knowledge in innovative ways
  • Interact with and contribute to the professional development of peers and colleagues
  • Serve as a leader in the practice setting and in the profession
  • Acquire and maintain current knowledge and competency in nursing practice
  • Consider factors related to safety, effectiveness, cost, and impact on practice in planning and delivering care

Required Qualifications

  • Bachelor's degree in Nursing
  • Active Tennessee RN license
  • Valid Tennessee driver's license
  • Excellent communication and customer service skills
  • Demonstrated clinical abilities

Preferred Qualifications

  • Pediatric nursing experience

Physical Requirements

  • Heavy lifting, pushing, and pulling up to 50 lbs occasionally, 20-30 lbs frequently, and 10 lbs constantly; occasional lifting or moving of up to 150 lbs in sudden emergency situations
  • Frequent bending, walking, sitting, squatting, reaching, and standing
  • Keyboard and computer use and/or repetitive motions may be required
