Home Health Registered Nurse - Full Time

Advantage Home Health Services is seeking a full-time Registered Nurse to provide direct patient care in the home setting throughout Huntingdon, PA and surrounding areas. In this role, you will conduct comprehensive assessments, develop care plans, coordinate patient care, and provide health education to patients and families.

Responsibilities

• Conduct Start of Care comprehensive assessments and complete OASIS documentation in a timely manner
• Draft individualized care plans and reconcile patient medications
• Ensure orders for care are accurate and complete
• Coordinate care with patients and provide reports to the Case Manager
• Prepare accurate, orderly, and timely documentation
• Provide health care instructions to patients and families as appropriate based on assessment and care plan

Required Qualifications

• Active RN license in Pennsylvania or eligibility to take the NCLEX
