HOME HEALTH Registered Nurse - FT (13708)

This full-time home health nursing position involves evaluating and admitting patients to home care, developing individualized plans of care, and providing skilled nursing services in patients' homes. You will serve as a case manager, adjusting care plans in response to symptom changes and evolving patient needs. You will coordinate with physicians and community healthcare partners on behalf of patients and caregivers, participate in scheduled case conferences, and manage all related documentation including assessments, start-of-care, and recertification paperwork.

Responsibilities

• Evaluate and admit patients to home care services
• Develop and implement individualized plans of care
• Provide direct nursing care according to established care plans
• Act as case manager and adjust care plans based on patient symptom changes and care needs
• Participate in scheduled case conference meetings to review and plan patient care and discharge
• Serve as liaison between patients, caregivers, physicians, and community healthcare partners
• Manage mileage, supplies, and visit scheduling to optimize cost of care
• Complete all documentation related to patient contact, assessments, start-of-care, and recertifications
• Manage resumption of care and discharge according to policy
• Maintain current knowledge of home care regulatory guidelines
• Provide on-call coverage for both home care and hospice as assigned
• Promote team behavior and exceptional patient and guest experiences
• Uphold and promote patient safety and quality standards

Required Qualifications

• Associate's degree in nursing
• Current Alabama RN license
• Minimum 2 years of experience in a hospital setting in medical-surgical, critical care, or emergency department nursing
• Strong critical thinking and organizational skills
• Ability to work independently and make independent decisions
• Flexibility with scheduling
• Ability to organize and prioritize a schedule to meet patient needs

Preferred Qualifications

• Previous home care or hospice experience
