Home Health Registered Nurse

Ascension at Home together with Compassus is seeking a Home Health Registered Nurse to support patient recovery and enable patients to remain in their homes while spending quality time with loved ones and engaging in activities they enjoy. In this role, you will provide clinical care, educate patients and families on disease management, and case manage patients while providing clinical oversight to an interdisciplinary team.

Responsibilities

• Support patient recovery and ability to stay at home
• Empower patients and families to reach healthcare goals through education on disease management
• Case manage and provide clinical oversight to interdisciplinary team members

Required Qualifications

• Active RN license in the state where you work
• Two years of experience as a registered nurse
• Current driver's license
• Current CPR and BCLS certification
