Home Health Registered Nurse Clinical Manager - Full Time On Site

We are seeking a Registered Nurse Clinical Manager to lead our Home Health team on a full-time, on-site basis. In this clinical leadership role, you will serve as the clinical leader of our operations, overseeing a field team of nurses while ensuring exceptional patient care and regulatory compliance.

Responsibilities

• Mentor and support a dedicated field nursing team while fostering a patient-centered culture
• Oversee clinical documentation, OASIS assessments, and regulatory compliance
• Partner with physicians, families, and referral sources to optimize patient outcomes
• Monitor quality metrics and lead performance improvement initiatives
• Work in the field as needed to support clinical operations

Required Qualifications

• Active RN license in Michigan
• Home Health experience required
• Solid understanding of OASIS and home health regulations
• Strong clinical judgment and organizational skills
• Excellent communication and team-building abilities
• Ability to multitask and manage competing priorities
• Ability to work in the field as needed

Preferred Qualifications

• OASIS experience
• Leadership or supervisory experience
