Home Health Registered Nurse - Central San Antonio

Home Care Providers of Texas is seeking a full-time Registered Nurse for home health care in the San Antonio, TX area. In this role, you will deliver skilled nursing care to patients in their homes, working with an interdisciplinary team to develop and implement individualized care plans. You will use the Roger app, an AI-driven, voice-enabled documentation platform designed to reduce charting time by 50% while maintaining OASIS compliance, allowing you to focus more on direct patient care.

Responsibilities

• Provide high-quality, comprehensive skilled nursing care in patients' homes according to physician orders and individualized plans of care
• Partner with the interdisciplinary care team to develop, implement, and update personalized care plans addressing each patient's unique needs
• Administer nursing treatments and manage clinical activities to support optimal health outcomes
• Continuously assess patient progress and the effectiveness of services; communicate findings with the care team, patient, family, physician, and case manager
• Perform additional duties as needed

Required Qualifications

• Active Texas RN license or Compact license
• Active CPR certificate
• Prior experience in geriatric home health care
• Valid driver's license
• Self-starter with strong organizational skills
