Home Health Registered Nurse Case Manager

Ascension at Home, together with Compassus, is seeking a Home Health Registered Nurse Case Manager to support patient recovery and enable patients to remain in their homes while spending quality time with loved ones and engaging in activities they enjoy.

In this role, you will case manage and provide clinical oversight to an interdisciplinary team while empowering patients and families to reach their healthcare goals through education on disease management.

Responsibilities

• Support patient recovery and ability to stay at home

• Case manage and provide clinical oversight to interdisciplinary team

Required Qualifications

• Active RN license in the state where you work
• Two years of experience as a registered nurse
• Current driver's license
• Current CPR and BCLS certification
