We are seeking a Registered Nurse to provide skilled nursing care in patients' homes on a PRN basis. This role focuses on delivering comprehensive home health services, including wound care, infusion therapy, catheter management, and patient education. You will conduct patient assessments using the OASIS system, develop individualized care plans, and maintain detailed documentation in compliance with federal and state regulations.
Responsibilities
- Conduct thorough initial and ongoing patient assessments using the OASIS tool to develop individualized care plans
- Provide skilled nursing care, including wound care, infusion therapy, catheter management, and other specialized services as prescribed
- Educate patients and families on managing health conditions, medications, and preventative care
- Maintain accurate, detailed, and timely documentation of patient conditions, interventions, and progress in compliance with federal, state, and agency guidelines
- Submit OASIS documentation promptly to ensure compliance with Medicare and agency standards
- Communicate changes in patient status to the care team and implement appropriate interventions
- Serve as a liaison between the patient, their family, and the care team to address questions and concerns
- Adhere to best practices and evidence-based protocols to ensure the highest standard of care
Required Qualifications
- Current and unencumbered RN license in Texas
- Minimum of 1 year of home health care nursing experience
- Proficiency in OASIS documentation
- Experience in wound care and infusion therapy
- Experience administering medication via PEG tube (percutaneous endoscopic gastrostomy)
- Knowledge of Kinser EMR
- Strong communication skills and ability to build rapport with patients and families
- Commitment to ongoing professional development
Preferred Qualifications
- Bachelor's degree in nursing (BSN)
Schedule
