Home Health PRN RN - Registered Nurse

Home Care Solutions is seeking a Registered Nurse (RN) to provide nursing care to patients in their homes throughout the Cookeville, Tennessee area and surrounding communities. This is a PRN (as-needed) position.

Position Overview

As an RN, you will provide and direct the provision of nursing care to patients in their homes as prescribed by their physician, in compliance with applicable laws, regulations, and agency policies. You will work independently and collaboratively to ensure continuity of care and positive patient outcomes.

Responsibilities

• Coordinate the total plan of care with other healthcare professionals involved in patient care
• Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources
• Make initial and comprehensive nursing evaluation visits
• Determine the primary focus of care and develop plans of care within state-specific guidelines
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer
• Maintain and submit accurate documentation and clinical notes, including patient condition, appropriate goals, interventions, and patient response to home health care
• Provide high-quality clinical services within the scope of practice as defined by state nursing laws, from admit through discharge

Required Qualifications

• One year of clinical RN experience
• Current RN license in the state where services are rendered
• Current CPR certification
• Valid driver's license
• Vehicle insurance
• Access to a dependable vehicle or public transportation
• Ability to reliably commute to multiple patient residences daily within the assigned geographic area

Work Environment

This position requires travel to multiple patient homes daily. You must have reliable transportation and the ability to commute throughout your assigned geographic area.
