Home Health or Hospice Registered Nurse

AdvisaCare is seeking a Home Health or Hospice Registered Nurse to provide direct patient care in a one-on-one setting across Traverse City, Kalkaska, Baldwin, Cadillac, Manistee, Gaylord, and surrounding areas. This role offers both part-time and full-time opportunities to work with patients and families during critical periods of care.

Responsibilities

• Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs
• Implement individualized plans of care and revise as needed based on patient status
• Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes and participate in interdisciplinary team conferences
• Participate in home health and hospice performance improvement programs
• Maintain high levels of customer satisfaction with clients, families, and caregivers
• Communicate information using current organizational processes and technology
• Ensure compliance with AdvisaCare policies, procedures, and Medicare requirements

Required Qualifications

• Current Michigan RN license in good standing
• Minimum one year of home health and hospice experience
• Ability to participate in daily field visits and weekly office meetings as indicated
• Ability to adapt to patients' changing status and needs
• Empathetic approach to patients and caregivers
