Home Health On Call Nurse 7 on 7 off

Ascend Home Health is seeking an experienced Registered Nurse (RN) to provide on-call skilled nursing support to patients throughout Essex and Bergen Counties, NJ during evening and overnight hours. This role involves responding to patient and caregiver needs, completing home visits when clinically necessary, assessing changes in patient condition, and coordinating care with physicians and the healthcare team.

Schedule

• 7 days on, 7 days off rotation
• On-call hours during assigned schedule
• Evening and overnight availability required

Clinical Setting

• Home health care in Essex and Bergen Counties, NJ
• Patient population served in home settings

Responsibilities

• Assume responsibility for assigned patient visits and on-call schedule, including assessment, planning, implementation, and evaluation of nursing care
• Respond to patient and caregiver calls during assigned on-call hours and determine appropriate clinical intervention
• Complete skilled nursing home visits as needed throughout the assigned territory
• Assess patients' physical and clinical status and identify changes requiring intervention or escalation
• Regularly re-evaluate patients' nursing and home health needs
• Provide skilled nursing care in accordance with physician orders and individualized plans of care
• Communicate changes in patient condition to the physician, clinical supervisor, and care team members
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals
• Provide patient and caregiver education regarding medications, treatments, disease management, safety, and the plan of care
• Administer medications and treatments in accordance with physician orders, federal and state regulations, and agency policies
• Perform skilled nursing procedures and treatments within the RN scope of practice
• Maintain accurate, timely, and complete clinical documentation at the point of service in accordance with agency policies and Medicare requirements
• Maintain regular communication with direct supervisor regarding assignments, patient concerns, and clinical matters
• Identify patient needs for additional services, medical equipment, supplies, or resources and coordinate appropriately
• Participate in on-call responsibilities in accordance with agency policies and procedures
• Maintain patient confidentiality and comply with HIPAA and applicable regulatory requirements

Travel Requirements

• Valid driver's license and reliable transportation required
• Must be available to travel locally throughout Essex and Bergen Counties during assigned on-call hours
• Mileage reimbursement provided

Required Qualifications

• State Certified RN license
• Experience in either hospice or home health care (required)
• Excellent documentation skills
• Ability to work independently
• Strong communication skills
• Valid driver's license and reliable transportation

Education

• Associate's Degree in Nursing
