The opportunity

Job Description

Safe Life Home Health, a Medicare-certified home health agency serving the greater Chicagoland area, is seeking a Home Health Office Nurse to serve as a central clinical point of contact supporting field clinicians, patients, families, physicians, and leadership. This full-time, on-site position in Lombard, IL is ideal for a detail-oriented nurse who thrives in a fast-paced environment and is committed to keeping clinical operations running smoothly while ensuring safe, high-quality patient care.

Responsibilities

  • Coordinate patient care across disciplines (skilled nursing, PT/OT/ST, MSW, HHA) to ensure continuity, timeliness, and adherence to the Plan of Care
  • Triage incoming patient calls, clinician calls, and physician communications; assess urgency, escalate appropriately, and document outcomes
  • Manage lab orders, results review, and follow-up; communicate critical and abnormal results to the case manager and ordering physician, and ensure documentation is closed out in the EMR
  • Support clinical leadership with day-to-day clinical operations, including care plan oversight, visit utilization tracking, and follow-up on outstanding orders, signed POCs, and physician communications
  • Process referrals, verify insurance and eligibility, schedule SOC visits, and assist the scheduling team with clinician assignments as needed
  • Serve as a clinical resource for field staff on documentation questions, supply needs, infection control, medication reconciliation, and patient-specific concerns
  • Communicate professionally and clearly with physicians' offices, hospitals, SNFs, and other referral sources
  • Support quality and compliance initiatives, including responding to documentation gaps and assisting with chart follow-up under the direction of clinical leadership
  • Maintain accurate, timely, and HIPAA-compliant documentation in the EMR
  • Perform other clinical and operational duties as assigned to support the team

Required Qualifications

  • Active Illinois RN or LPN/LVN license in good standing
  • Minimum 2 years of home health experience
  • Proven hands-on experience with Kinnser/WellSky EMR
  • Strong working knowledge of Medicare home health requirements and home health workflow (referral, SOC, recertification, discharge)
  • Ability to work effectively under pressure, manage competing priorities, and meet deadlines without compromising accuracy or patient safety
  • Demonstrated commitment to quality of care and patient safety
  • Self-starter who takes initiative and ownership with reliable follow-through on assigned tasks
  • Fast learner comfortable picking up new processes, tools, and payer requirements quickly
  • Excellent communication, organizational, and time-management skills
  • Positive, professional demeanor and ability to collaborate effectively with clinicians, office staff, physicians, and families

Schedule

  • Full-time, on-site position at Lombard, IL office
  • 40-45 hours per week

Compensation and Benefits

  • Hourly pay: $30.00-$40.00 per hour, based on experience and licensure
