Carilion Clinic is seeking a full-time Home Health Nurse - Wound Specialist to provide comprehensive, patient-centered nursing care in the home setting throughout the Roanoke region. In this role, you will coordinate and deliver individualized care plans across the continuum of care, utilizing clinical expertise, critical thinking, and collaboration to promote optimal patient outcomes and support safe, effective care transitions.
Responsibilities
- Assess, plan, implement, and evaluate patient care to achieve optimum outcomes for patients of all ages
- Complete age-specific admission and ongoing assessments, document findings, and develop individualized plans of care
- Collaborate with the healthcare team, patients, and families to develop, implement, and evaluate interdisciplinary plans of care, revising as needed based on changes in patient status
- Accurately administer and document treatments and medications using automated computer systems and electronic medical records
- Identify patient and family educational needs in a timely manner, incorporate them into the plan of care, and document appropriately
- Serve as a patient advocate and protector of patient rights, providing psychological, social, and cultural support
- Apply knowledge of growth and development to appropriately care for patients served
- Contribute to performance improvement activities

- Full-time, Day/Evening shift

Required Qualifications
- Bachelor of Science in Nursing (BSN)
- Minimum of three years of clinical nursing experience
- Currently licensed as a Registered Nurse in the Commonwealth of Virginia
- Valid Virginia Driver's License
- Pass a Criminal Record Check as required by Virginia state law
- American Heart Association BLS HCP certification (must obtain and maintain)
- Keyboarding skills for automated computer systems and electronic medical records
- Ability to meet essential job functions
- Successful completion of orientation and competency requirements
Preferred Qualifications
- Wound and ostomy certification
Compensation and Benefits
- Salary range: $76,689.60 - $115,044.80 annually (based on qualifications, credentials, education, and prior work experience)
