Altruist Home Care Solutions is seeking a Home Health Nurse RN to provide comprehensive nursing care to patients in the home setting within the Dallas-Fort Worth and East Texas region. You will manage a caseload of patients, perform clinical assessments, implement treatment plans, and coordinate care with the interdisciplinary healthcare team. This role requires travel within a 30-mile radius of the Greenville location at 5403 Wesley St. Suite B, Greenville, TX 75402.
Clinical Setting
You will deliver home health and hospice nursing services, working directly with patients in their homes. The role includes consultation with physicians and office staff regarding patient care details.
Responsibilities
- Assess and evaluate patient cases and prepare and implement treatment and case management plans
- Perform accurate OASIS collection and ensure medication profiles remain current
- Ensure lab values are reported to the physician in a timely manner
- Attend weekly case conferences and case manager meetings
- Prepare and maintain written case records, reports, and forms
- Implement and coordinate patient care and perform administrative tasks as required
- Confer with health team members to exchange, discuss, and evaluate patient information for planning, modifying, and coordinating treatment programs
- Explain procedures and treatments to patients to gain cooperation and understanding
- Administer prescribed medications and treatments in accordance with physician orders and approved nursing techniques
- Maintain awareness of comfort and safety needs of patients
- Observe patients, record significant conditions and reactions, and notify supervisor and/or physician of patient condition and reactions to drugs, treatments, and significant incidents
- Take temperature, pulse, blood pressure, and other vital signs to detect deviations from normal
- Respond to life-saving situations based upon nursing standards, policies, procedures, and protocol
- Document nursing history and physical assessment for assigned patients
- Initiate patient education plans according to individualized patient needs as prescribed by physician and/or company policy
- Participate in quality improvement activities
- Complete all documentation within 24 hours of visits, 7 days per week
Required Qualifications
- Current and valid Texas RN license
- Minimum 2 years of nursing experience
- Minimum 1-2 years of hospice experience
- Current and valid Texas driver's license
- Proof of current and valid auto insurance
- Proof of TB skin test result
- Impeccable work history with professional references
- Willingness to see patients in the assigned coverage area
- Proficiency with OASIS admissions for home health patients
- Excellent interpersonal skills and ability to communicate effectively with the Altruist team, patients, and referral sources
- Ability to commute to the Dallas office and be onsite (this is not a hybrid role)
- Personally owned automobile that is insured in accordance with state and organization requirements and is in good working order
- Must be a licensed driver
Preferred Qualifications
- 1-2 years of home health experience
- Working knowledge of various EMR systems such as Careficient, Winnolas, or HomeCare Homebase
Education
- Bachelor's degree in healthcare administration, nursing, or a related field
- Strong understanding of healthcare terminology, regulations, and practices
Licenses and Certifications
- Current and valid Texas RN license
- Current CPR certification
- Current and valid Texas driver's license
- TB skin test result
